If anything changes after you submit this form, you can email [email protected] to update something you’ve provided here or if you have any questions! iPads will be available at check-in to complete this form if you do not fill it out before you arrive, but it will save you time to do it now. This form is for children in Kids Camp only (ages 0-2, 3 & 4, and 5 & 6, or fall kindergarteners and first graders).
