A new article from Michigan Medicine Health Lab explores strategies to reduce hospital readmissions among patients with type 2 diabetes by addressing social determinants of health. The initiative, called the Re-Engineered Discharge for Diabetes Care Transitions (REDD-CAT2), is led by Noelle Carlozzi, professor of physical medicine. REDD-CAT2 screens for unmet social needs and offers support through a patient navigator after discharge. Could a simple post-hospital intervention be the key to better outcomes for diabetes patients facing barriers like low income or limited access to care?
